Understanding Preventive Care

Preventive care includes routine wellness exams, screenings, and immunizations intended to prevent or avoid illness or other health problems, and are covered at no cost to a plan member when provided by an in-network provider.

When you stay up to date on preventive care you have a better chance of catching emerging health issues early, when:

  • they are usually more treatable and less expensive to treat, and
  • have the potential for a better outcome.

Please note that the same procedure may be considered preventive or diagnostic depending on the situation. Learn more about the difference between preventive and diagnostic care.

Help avoid more serious condition-related events that can be expensive to treat and decrease your quality of life by:

  • Maintaining a relationship with your PCP
  • Managing any chronic conditions that you may have such as hypertension or diabetes
  • Staying compliant with your maintenance medications

Get Personal Support for Chronic Health Conditions

UnitedHealthcare provides a program called Personal Health Support designed to encourage personalized, efficient care for you and your covered dependents.

Personal Health Support nurses center their efforts on prevention, education, and closing any gaps in your care to ensure you receive the most appropriate and cost-effective services available.

If you are living with a chronic condition or dealing with complex health care needs, UnitedHealthcare may assign to you a primary nurse, referred to as a Personal Health Support Nurse, to guide you through your treatment answer questions, explain options, identify your needs, and/or refer you to specialized care programs, if appropriate.

UnitedHealthcare’s Condition Management Program

If you have been diagnosed with CAD, COPD, heart failure, asthma or diabetes, you are eligible to participate in this program.

Important Note Regarding the Surgery Decision Support Program

If you are enrolled in one of The Hartford’s medical plan options, please keep in mind that a $1,000 penalty will be imposed if a participant has one of the five surgeries listed below on an elective basis and does not complete the requirements of My Medical Ally's Surgery Decision Support (SDS) program at least 30 days prior to having the surgery.

  • Bariatric (weight loss) Surgery
  • Hysterectomy
  • Knee Replacement
  • Hip Replacement
  • Low Back Surgery

What is the difference between a Virtual Visit and a telehealth visit?

A Virtual Visit is a medical or behavioral health visit conducted over the internet or on the telephone by one of UHC’s Virtual Visit vendors. A telehealth visit is a medical or behavioral health visit conducted over the internet or on the telephone by your PCP or other healthcare provider. A telehealth visit with your PCP or other healthcare provider will be covered the same as any office visit. Virtual visits with UHC’s vendors are generally less expensive than an office visit.

Behavioral Health Support

All employees and their household members, including kids at college are eligible to receive – at no cost – ten confidential counseling sessions per year through our Employee Assistance Program. Learn more.

If you are covered by The Hartford’s health plan, have a need for:

  • Longer-term outpatient care
  • Medication management
  • Special needs/preferences

You can access UHC’s Behavioral Health resources through the Mental Health Service link on myUHC.com. Additional support and providers are available through Lyra on thehartford.lyrahealth.com.
